Community Support Requests

The Iowa Clinic strives to be a good citizen as well as support the people who live in the communities where we do business. 

Requests for funding will be evaluated based on the strategic business objectives of The Iowa Clinic with a priority to not-for-profits supporting these key focus areas:

• Health & Wellness – 75%
• Basic Needs – 25%

Due to the volume of proposals received, not all requests will be funded. Organizations may only submit one request per year, so the request should include all events and programs using the form below. No emailed or mailed requests will be accepted.

IMPORTANT: Please use one of the three upload file fields below to upload your W9. 

Requests are reviewed quarterly and must be submitted by the following dates:

Q1 – March 31
Q2 – June 30
Q3 – September 30
Q4 – December 31

A response will be provided within 30 days following the deadlines listed above. If you have any questions or need further clarification, please email info@iowaclinic.com.
